35Y/F with viral pyrexia and thrombocytopenia

A 35yr old female patient came to the casualty with c/o fever since 3 days joint pains and burning sensation of epigastric region since 3 days
HOPI -
Patient was apparently asymptomatic 3 days back then she developed high grade fever associated with chills and joint pains
No H/o vomitings, loose stools ,black colored stools, SOB, cold, cough, giddiness, hematuria
No rash & bleeding manifestations .
No H/o retro orbital pain , joint pains .
Past History -
Not a k/c/o HTN , DM,TB, Asthma , CAD, CVD
Personal history -
Diet - mixed
Appetite - Normal
Bowel & bladder movements - regular
No addictions
Family history - not significant
On Examination
patient is c/c/c
No pallor , icterus , cyanosis , clubbing lymphadenopathy , edema
Vitals -
Temp - Afebrile
PR - 78bpm
BP - 130/80 mmHg
RR -20 cpm
SpO2- 98%@RA
Systemic examination -
CVS - S1 S2 +VE
RS -BAE +VE , NVBS +VE
P/A - soft , non tender , BS +ve
CNS - NAD
Provisional diagnosis -
Viral pyrexia
with Thrombocytopenia
TREATMENT:
IVF NS/RL/DNS @100ML/HR
INJ.PANTOP 40MG IV BD
INJ.ZOFER 4MG IV SOS
INJ.OPTINUERON 1AMP IN 100ML NS IV OD
TAB.PCM 650MG PO TID
INJ.NEOMOL 1GM IV SOS (IF TEMP >101°F)
PLENTY OF ORAL FLUIDS
TEMPERATURE CHARTING 4th HOURLY
